An administrative fee of $47.50 is to be submitted along with this request.
Your degree or diploma is a legal document and therefore you may possess only one original parchment.
If you are requesting a replacement of your original, (eg. due to damage or name change), we require the original returned. If you are no longer in possession of your original, we must receive written confirmation with the official seal of a Notary Public stating that you have lost/misplaced your degree or diploma.

If you are requesting a copy of your degree or diploma, all that is required is the completion of this form and payment. The degree/diploma will have "DUPLICATE" printed on the parchment.
Please allow a minimum of three weeks for processing (longer during Convocation periods in May, June, and October).
You will receive an email with either pick up instructions or a courier tracking number once the degree is ready.
